What should have been a simple statutory oath request, turned into a refusal from Mr Happyman due to not being happy where the signature, name and address was placed at the end of the form. Twice he told the secretary to say I had to provide an English format. When I asked for clarification on what that meant as the document was in English and contained the wording needed to deem it a statutory declaration, he decided to come out of his office and cross out where I had put name and address at the bottom of the page. He only needed to sign and stamp the area, but refused to do it, because of the name and address written there. I researched extensively to ensure the wording was correct and used notary.co.uk and notarypublicinlondon.com. This last site had an example template of a statutory declaration.
I can only haphazard a guess at such pedantic behaviour - a relatively cheap service and just couldn't be bothered with the effort.
